Immunization requirements at FLC

Colorado State Law requires you to submit an immunization record with evidence of two MMR vaccinations (measles, mumps and rubella) to the Student Health Center. This is a mandatory requirement for all college students born on or after January 1, 1957. Colorado State Law also requires Fort Lewis College to provide information to all incoming freshman students living on campus about meningococcal (meningitis) disease. 

A current copy of your immunization record must be submitted once by: census date in the fall, spring or summer. If you do not comply with this requirement, a hold on making any registration changes will be placed on your student account. 

How-to upload records

Immunization records can be uploaded to the Student Health Portal, theFort, or FLC Student Health Center website. 

  1. Log in using your CAS information
  2. Choose My Forms and select Immunizations
  3. Enter the dates of your MMR vaccines to be considered compliant
  4. Upload a copy of your vaccine record and select Submit

Please note that compliance will take up to 24 hours to update.

Immunization compliance is required to register for classes. Do you need a vaccine? Vaccines are available by appointment at your Student Health Center. MMR titers (a simple blood test to show immunity) are also available by appointment. If you have an existing MMR titer, please upload a copy of the lab work under the Upload Documents tab on the Student Health Portal link above.

Request an exemption

You have the legal right to sign an exemption from receiving this vaccination. If you do so, you will be subject to quarantine and excluded from campus for up to 3 months in the case of an outbreak of measles, mumps, or rubella.

  1. To submit an exemption, file the Non-Medical Exemption Form WITH a signature from an immunizing provider, OR
  2. Complete the online education module and submit the Certificate of Completion with the Non-medical Exemption Form
  3. Use the Documents Upload tab on the Student Health Portal link above to upload your Certificate of Completion and Colorado Nonmedical Exemption Form.
  4. Submit your immunization records by the Census Date. Call the Student Health Center 970-247-7355 if you need assistance obtaining your records.
