Read the Kitchen Policy
Please list the date and time that the kitchen will be in use (include set up and clean up time).
In the event that your first choice isn't available, please list the alternative date and time that you would like to use the kitchen (include set up and clean up time).
Your initials indicate you understand that the kitchen must be left as clean, if not cleaner than it was before the event.
I understand that a Staff member of El Centro or The Native American Center must check out the kitchen before I leave. I agree to clean the Kitchen to the standards established in The Kitchen Checklist or risk losing Kitchen privileges (please initial below).
As a Commercial Kitchen licensed and inspected by the State of Colorado, law requires that at least one person using the facility has completed Safe Serve Training.
