Incident Reporting at FLC

What is an incident?

  • An incident is a safety or health event with unwanted consequences. They can be large scale (campus) or small scale (individual). It occurs at work or while working.
  • “Near misses” are incidents too, only they are about what could have happened. They still need to be corrected promptly.
  • All incidents are confidential and subject to HIPPA laws.

How do I report an incident?

  1. ALWAYS REPORT to your supervisor first and immediately, if immediate emergency medical attention is needed, please proceed to Mercy Medical Center and then notify your supervisor.
  2. Supervisors must report to EHS and HR immediately.
  3. Supervisor reports to EHS

  4. EHS will follow up with Supervisors with the following:
    1. Incident report for supervisor and injured person to fill out, take photos, witness form
    2. EHS assistance in filling these out
    3. Debrief and corrective/preventive action
  5. If the injury is NOT an EMERGENCY to you or another person, notify Trish Grogan or HR to begin the claims process for Worker’s Compensation.

  6. DO NOT go to your personal physician, Worker’s Compensation will not cover it, you must go to a Worker Compensation facility on Page 2.

If this is a chemical exposure: 

  1. Call Supervisor and EHS immediately
  2. SDS look up is at this link: https://chemicalsafety.com/sds-search/
  3. Perform first aid measures from SDS
  4. Go to Medical Facility immediately
    1. Urgent Care (Non-life threat)
    2. Emergency Room (Life Threat) - Call 911

Why do we report incidents?

  1. To ensure any injured person is taken care of quickly and with the best possible care.
  2. To identify safety hazards and correct them
  3. So EHS can look at trends, train on them and assist in mitigating the issues, creating a safer campus for everyone.

Will I get in trouble if I get hurt?

  1. NO! That would not be legal or ethical
  2. We want to ensure our people are getting the care they need ASAP.
  3. We want to remove the hazard or mitigate it ASAP.
